Early Ejaculation Therapy: Control Exercises, Pelvic Floor Training and Counselling

Control exercises, pelvic floor training and counselling are first-line therapy for premature ejaculation. Here is how each one works and how to practise it.
What is early ejaculation therapy?
Early ejaculation therapy means non-drug treatments that teach the body and mind to delay ejaculation. Doctors usually recommend them first because they are safe, cost nothing and stay with you once learned. They can be used on their own or alongside medicine or Unani treatment.
The three pillars are control exercises (stop-start and squeeze), pelvic floor exercises, and counselling for anxiety and relationship factors. Most men need four to twelve weeks of regular practice to notice clear improvement.
Stop-start and squeeze methods
These methods train you to recognise the “point of no return” - the moment just before ejaculation becomes inevitable - and to bring arousal down before you reach it.
- Stop-start technique: during self-stimulation or with your partner, stimulate until you feel close to climax, then stop completely for 30-60 seconds until the urge fades. Repeat three or four times before allowing ejaculation.
- Squeeze technique: when you feel close, you or your partner gently squeeze the head of the penis where it meets the shaft for 10-20 seconds, then wait 30 seconds before continuing.
- Practise three times a week. Start alone, then with your partner, and finally during intercourse with slow, controlled movement.
- Breathe slowly and deeply - shallow, fast breathing speeds up arousal.
Pelvic floor exercises for premature ejaculation
The pelvic floor muscles control ejaculation. Strengthening them with Kegel exercises gives you more voluntary control. Studies of pelvic floor training in men with lifelong premature ejaculation have reported meaningful improvement in many participants after about 12 weeks.
- Find the muscles: they are the ones you would use to stop urine mid-flow (do this only once to identify them, not as a regular exercise).
- Squeeze for 3 seconds, relax for 3 seconds. Repeat 10 times.
- Do three sets a day - lying down, sitting and standing.
- Increase to 5-10 second holds over a few weeks. Keep your stomach, buttocks and thighs relaxed.
Counselling for premature ejaculation
Anxiety is both a cause and a result of early ejaculation: worry speeds up ejaculation, which creates more worry. Counselling breaks this cycle. A counsellor or experienced doctor helps you understand what is normal, change unhelpful thoughts (“I will fail again”) and reduce performance pressure.
Couple counselling is especially helpful. Sensate focus - a structured programme of non-demanding touch where intercourse is temporarily off the table - lowers anxiety and builds communication.
Lifestyle steps that support therapy
- Sleep 7-8 hours; tiredness lowers control.
- Limit alcohol and stop smoking.
- Exercise regularly - it reduces anxiety and improves pelvic blood flow.
- Use a thicker condom, which can reduce sensitivity for some men.
- Avoid “timing” sprays or tablets bought online without a prescription.
How Unani medicine fits in
At Hashmi Dawakhana, Amroha, we combine these methods with individually prescribed Unani treatment for premature ejaculation (Surat-e-Inzal), diet and routine guidance. Therapy builds control; Unani care addresses underlying weakness and imbalance as understood in Unani medicine.
When to see a doctor
Consult a doctor if early ejaculation has lasted more than a few months despite practice, if it started suddenly, or if it comes with erection problems, pain or urinary symptoms.
